♣ 식도 식도를 관찰할 때는 가능한 NBI를 사용하고(brownish color change 이상소견 가능성↑), 식도의 혈관상이 소실된 부분을 주의 깊게 관찰한다. Withdrawal시, 끝까지 송기하면서 마지막까지 관찰을 주의깊게 한다. Sentinel polyp에서 cancer가 발견되는 경우가 종종 있으므로 가능하면 biopsy한다. ♣ 위위 병변을 놓치지 않기 위해서, 일관된 관찰 습관을 유지하고 충분한(최소 3분 이상, 4분 이상 권고) 시간 동안 관찰한다. ▶함몰이나 궤양이 주변 점막 주름의 집중이나, 단절, 융합 등의 소견을 보일 때, ▶함몰의 형태가 부정형의 모습을 보이고 국소적인 재생상피가 관찰될 때, ▶저부의 백태가 주변을 삐져나오고, 저부가 평탄하지 않고 섬모양의 점막 등을 관찰할 ..

몇 가지 기억에 남는 것들 정리. 첫째 날. 1. 미다졸람(Midazolam)시작용량은 0.03mg/kg, shooting하지말고 천천히 주사. 최대용량은 5-6mg. 65세 이상인 경우 20% 감량. 미다졸람은 천정효과(Ceiling effect)가 있어, 용량을 높이더라도 진정의 깊이가 깊어지지 않고, 회복시간만 길어짐. 약제 투여 직후부터 난폭해지면 paradoxical response, 시술 도중 난폭해지면 통증으로 인한 회피반응일 가능성이 크다. 2. 프로포폴(Propofol) 프로포폴에는 soybean oil이 10% 가량 포함되어 있는데 이 때문에 혈관통이 있고, 알러지가 있을 수 있으며, 세균이 자라기 쉽다. 또한 생산된 날짜에 따라 진정효과가 다를 수 있어, 병원에 납품된 날짜에 따라 환..

감기 nasopharyngitis 1) 진단 : 콧물, 코막힘, 인후통이 있을 때, 충분한 문진과 진찰을 통해 다른 유사 호흡기 질환을 배제하고 감기(nasopharyngitis)로 진단한다. 2) 치료 : 감기의 원인은 대부분 바이러스이므로 감기의 치료에 항생제를 사용하지 않는다. 3) 설명 : 감기로 진단한 경우, 자연 치유되는 감기의 경과에 대해 설명한다. 단, 합병증을 동반할 수 있으므로 경과 관찰이 필요함을 설명한다. 4) 항생제 고려 : ①39˚C 이상의 발열, ②화농성 콧물, ③안면 통증, ④10일 이상 지속되는 증상, ⑤호전되다가 악화되는 경우에는 세균성 부비동염 의심하에 항생제 사용을 고려한다. 5) 재평가 : 감기 치료 중 10일이 지나도 증상 호전되지 않거나, 그 이전이라도 증상의 심..
